Physical Therapy is needed when... Functionality - A decrease is noticed in someone's functional status (ie: unable to walk anymore, unable to transfer from sit to stand anymore, cannot get in or out of bed independently, and any change in function). Never accept these conditions as "normal". Activities/Tasks - If family members indicate that the individual was recently more independent. Transfers - Nursing staff has difficulty assisting a patient with transfers, ambulation, and general mobility. Dr. Prescription - You have a Doctor's order for a PT evaluation. Balance/Walking - You notice that an individual has difficulty with balance and experiences increased incidents of falling.Physical Therapy provides restoration of normal or as near normal functioning in an individual through the use of Physical modalities, exercise, and hands-on treatment. Occupational Therapy is needed when... Meals - Individual has difficulty handling utensils, spilling or dropping food, and inappropriate behavior. Dressing/Grooming/Bathing - Individual has difficulty fastening fasteners, putting on or pulling off socks and or shoes, and difficulty reaching body areas. Activities/Tasks - Individual has weakness in arms or hands, does not understand how to do activity, is not alert or focused for activities, behaves inappropriately such as restlessness, wandering, or acting out. Transfers - Individual has difficulty transferring to or from wheelchair, toilet, bed, chairs, tub/shower or car. Sitting/Positioning - Individual has difficulty sitting or positioning self in chair or wheelchair. Splints - Individual has difficulty with opening of using hands (splint may be needed).Home visits for safety purposes (ie: guarding against falls, bathroom safety and phone and emergency plans) may be appropriate.

Speech Therapy is needed when... Meals - An individual has occurrences of coughing or choking when eating meals or snacks. Activities/Tasks - An individual has trouble remembering their daily routine or required tasks. Speech - An individual has trouble being understood by family or staff. Communication - An individual has trouble verbalizing and communicating what they want. Speech therapy is available for children and adults in our clinics, in a partner facility, or in the home.
Social Work (as related to therapy services) Tri-Therapy's Home & Community Rehabilitation Services program provides comprehensive rehabilitation and reintegration services in the comfort of your own home and community.
Reintegration into an individual's community following a traumatic injury or discharge from a hospital or rehabilitation facility. Job placement assistance after life-altering physical event. Transportation arrangements. Planning daily activities.
Licensed Social Workers - Tri-Therapy works with clients and families to establish a plan and activities to socially rehabilitate our clients.
